num_instances is initialized to 1 and overriden in ac_query_gpu_info.
has_graphics needs to be set early as other info fields are determined
based on its value.

The ordered atomic commits the post-add offset to memory, but overflow was computed using the pre-add offset, causing partial overflows to be missed and counters to become corrupted.
Fixes: "KHR-GL46.transform_feedback_overflow_query_ARB.multiple-streams-one-buffer-per-stream" based on the postwrite buffer offset, rather than the offset before the current workgroups writes.

The new definitions have their numbers offset by 1 (e.g. S_580 -> S_581).
The remaining old definitions are adjusted to match that.

AC_TRACKED_DB_PA_SC_VRS_OVERRIDE_CNTL can be used instead because
the DB and PA registers are mutually exclusive.
2 definitions are moved because consecutive enums aren't allowed
to cross a multiple of 32 because of static assertions in the bitset.

With the per-submit mode, the driver was overwriting the existing file.
To fix that properly, add a RGP capture mode to add _frameXXX or
_submitXXX to the filenames.

This keeps old packets that were removed from newer HW, packets that set
registers, and packets using non-trivial custom code.
It preserves address checking that was done in print_addr.
Packet names still used the old generator.
