4695bebc79 nir: Drop nir_dest
Instead, we replace every use of it with nir_def.  Most of this commit
was generated by sed:

   sed -i -e 's/dest.ssa/def/g' src/**/*.h src/**/*.c src/**/*.cpp

A few manual fixups were required in lima and the nir_legacy code.

01e9ee79f7 nir: Drop unused name from nir_ssa_dest_init

8bffd601ed Revert "nir: Preserve offsets in lower_io_to_scalar_early"
This reverts commit 96fa23bca5.

The correct fix to the problem was a1bc152340, making this
change obsolete as the pass skips any vars marked with
always_active_io. There was no real advantage to allowing these
vars to be split because they can't be removed anyway. Also there
is no way to split varying arrays gracefully here due to the xfb
layout rules, and this change didn't handle arrays at all.

Removing this obsolete code also fixes an assert in the new CTS
test KHR-Single-GL45.enhanced_layouts.xfb_all_stages. The test
was legally adding xfb offsets to all vertex stages but since
we only mark the varyings in the final vertex stage with the
always_active_io flag the other stages were correctly lowering
to scalars but when an array with an offset hit this code it
asserted since it couldn't handle it.

c1fbd0b8ab nir: skip lowering io to scalar for must_be_shader_input
These varyings cannot be packed by the GLSL linkers packing pass
so we need to skip this lowering until later when we can properly
handle them.

3cc58e6470 nir: Add and use some deref mode helpers
NIR derefs currently have exactly one variable mode.  This is about to
change so we can handle OpenCL generic pointers.  In order to transition
safely, we need to audit every deref->mode check.  This commit adds a
set of helpers that provide more nuanced mode checks and converts most
of NIR to use them.

For simple cases, we add nir_deref_mode_is and nir_deref_mode_is_one_of
helpers.  These can be used in passes which don't have to bother with
generic pointers and just want to know what mode a thing is.  If the
pass ever encounters generic pointers in a way that this check would be
unsafe, it will assert-fail to alert developers that they need to think
harder about things and fix the pass.

For more complex passes which require a more nuanced understanding of
modes, we add nir_deref_mode_may_be and nir_deref_mode_must_be helpers
which accurately describe the compiler's best knowledge about the given
deref.  Unfortunately, we may not be able to exactly identify the mode
in a generic pointers scenario so we have to be very careful when we use
these.  Conversion of these passes is left to later commits.

For the case of mass lowering of a particular mode (nir_lower_explicit_io
is one good example), we add nir_deref_mode_is_in_set.  This is also
pretty assert-happy like nir_deref_mode_is but is for a set containment
comparison on deref modes where you expect the deref to either be all-in
or all-out.

bb5d5029b7 nir: Use var->data.mode instead of deref->mode in a few cases
We already have the variable so we know the mode exactly.  Just use that
instead of the deref mode.  If these paths ever have to handle variable
pointers (not likely since they're OpenGL-specific), we can fix them to
handle crazy deref modes then.

0aa08ae2f6 nir: Split NIR_INTRINSIC_TYPE into separate src/dest indices
We're about to introduce conversion ops which are going to want two
different types.  We may as well just split the one we have rather than
end up with three.  There are a couple places where this is mildly
inconvenient but most of the time I find it to actually be nicer.

d80c342d89 nir: add deref lowering sanity checking
This will be removed at the end of the transition, but add some tracking
plus asserts to help ensure that lowering passes are called at the
correct point (pre or post deref instruction lowering) as passes are
converted and the point where lower_deref_instrs() is called is moved.

6af5e0bec9 nir: add variant of lower_io_to_scalar to be called earlier
This is intended to be called before nir_lower_io() so that we
can do some linking optimisations with the results. It can also
be used with drivers that don't use nir_lower_io() at all such
as RADV.

v2: pass mode mask rather than first and last stage integer.

0320bb2c6c nir: Use nir_src_copy instead of direct assignments.
If the source is an indirect register, there is ralloc'd data.  Copying
with a direct assignment will copy the pointer, but the data will still
belong to the old instruction's memory context.  Since we're lowering
and throwing away instructions, that could free the data by mistake.

Instead, use nir_src_copy, which properly handles this.

This is admittedly not a common case, so I think the bug is real,
but unlikely to be hit.
