Most of the time with nir_def_rewrite_uses_after, you want to rewrite after the
replacement. Make that the default thing to be more ergonomic and to drop
parent_instr uses.
We leave nir_def_rewrite_uses_after_instr defined if you really want the old
signature with an arbitrary after point.

Instead, we replace every use of it with nir_def. Most of this commit
was generated by sed:
sed -i -e 's/dest.ssa/def/g' src/**/*.h src/**/*.c src/**/*.cpp
A few manual fixups were required in lima and the nir_legacy code.

It's kind-of an anomaly that the Intel drivers are still treating
gl_FragCoord as an input. It also makes zero sense because we have to
special-case it in the back-end.
Because ANV is the only user of nir_lower_wpos_center, we go ahead and
just update it to look for nir_intrinsic_load_frag_coord as part of this
patch.

This is instead of looking all the way back to the variable which may
not exist for all derefs. This makes this code properly ignore casts
with modes other than the mode[s] we care about (where casts aren't
allowed).

plus asserts to help ensure that lowering passes are called at the
correct point (pre or post deref instruction lowering) as passes are
converted and the point where lower_deref_instrs() is called is moved.

According to section 14.6 of the Vulkan specification:
"When sample shading is enabled, the x and y components of FragCoord
reflect the location of the sample corresponding to the shader
invocation."
So add a boolean parameter to the lowering pass to select this behavior
when we need it.

Otherwise we rewrote the fadd to use itself, causing crashes in
validation. Instead, start after the last use like we should.

nir_lower_wpos_ytransform() is great for OpenGL, which allows
applications to choose whether their coordinate system's origin is
upper left/lower left, and whether the pixel center should be on
integer/half-integer boundaries.
Vulkan, however, has much simpler requirements: the pixel center
is always half-integer, and the origin is always upper left. No
coordinate transform is needed - we just need to add <0.5, 0.5>.
This means that we can avoid using (and setting up) a uniform.
I thought about adding more options to nir_lower_wpos_ytransform(),
but making a new pass that never even touched uniforms seemed simpler.
v2: Use normal iterator rather than _safe variant (noticed by Matt).
