While not currently required, it will be for future GPUs.
Also cleans up gpu_id as parameter to some functions that didn't use it.

This replaces all full lisence headers with SPDX identifiers and
generally makes things more consistent. I've also dropped the few
remaining author tags. If someone wants to know who wrote a bit of
code, `git blame` is going to be way more accurate than author tags
anyway.

Before this commit, if an application tried to create more than one
panfrost_device, such as piglit's "ext_image_dma_buf_import" tests,
it would result in a crash when running with PAN_MESA_DEBUG=sync or
PAN_MESA_DEBUG=trace.
This commit fixes that by introducing a pandecode_context, which
encapsulates all the information that is being tracked, and thus
avoiding memory conflicts.

This switches us over to Mesa's code style [1], normalizing us within the tree.
The results aren't perfect, but they bring us a hell of a lot closer to the rest
of the tree. Panfrost doesn't feel so foreign relative to Mesa with this, which
I think (in retrospect after a bunch of years of being "different") is the right
call.
I skipped PanVK because that's paused right now.

Introduces a binary dump analyser and BO decoder whose format was partially
borrowed from etnaviv's kernel driver. It leverages the pandecode library
to analyse BO's attached to the GPU job that caused the crash.
Binary dump should be generated by a component of Panfrost's kernel driver.
