692e1ab2c1 anv: get rid of the second dynamic state heap
Pretty big change... Sorry for that.

I can't exactly remember why I created 2 heaps. I think it's because I
mistakenly thought the samplers in the binding sampler pointers needed
to be indexed from the binding table. But that's not the case, they
just need to be in the dynamic state heap.

In the future, this change will allow to also allocate buffers for
push constant data in the newly created dynamic_visible_pool which
will be useful on < Gfx12.0 where this is the only place push constant
data can live for compute shaders.

1de44b1951 anv: add pipeline/shader support for descriptor buffers
Lowering/layout is pretty much the same as direct descriptors. The
caveats is that since the descriptor buffers are not visible from the
binding tables we can't promote anything to the binding table (except
push descriptors).

The reason for this is that there is nothing that prevents an
application to use both types of descriptors and because descriptor
buffers have visible address + capture replay, we can't merge the 2
types in the same virtual address space location (limited to 4Gb max,
limited 2Gb with binding tables).

If we had the guarantee that both are not going to be used at the same
time, we could consider a 2Gb VA for descriptor buffers.

7c76125db2 anv: use 2 different buffers for surfaces/samplers in descriptor sets
We had the unfortunate finding on a recent platform to learn that the
bindless sampler heap is not functioning as expected.

Nowhere in the documentation is the size of the heap written down. So
most people assumed that's the max number that we can program (4Gb).

The reality is that it's only 64Mb.

Though it is appearing like it's working properly for the whole 4Gb
range for most apps, this is only because the HW bounds checking
applied is broken. Instead of clamping anything beyong 64Mb, it's only
clamping the last 4Kb of each 64Mb region.

So this heap is useless for us to make a 4Gb region of both sampler &
surface states...

This change essentially turns off the bindless sampler heap on DG2+.

The only location where we can put SAMPLER_STATE elements is the
dynamic state heap. Unfortunately we cannot align the dynamic state
heap with the bindless surface state heap. So the solution is to
allocate sampler & surface states separately, each from the own heap
in the descriptor pool.

We now have to provide 2 sets of offsets for surfaces & samplers.

873dfb673b anv: Perform load_constant address math in 32-bit rather than 64-bit
We lower NIR's load_constant to load_global_constant, which uses A64
bindless messages. As such, we do the following math to produce the
address for each load:

   base_lo@32 <- BRW_SHADER_RELOC_CONST_DATA_ADDR_LOW
   base_hi@32 <- BRW_SHADER_RELOC_CONST_DATA_ADDR_HIGH
   base@64 <- pack_64_2x32_split(base_lo, base_hi)
   addr@64 <- iadd(base@64, u2u64(offset@32))

On platforms that emulate 64-bit math, we have to emit additional code
for the 64-bit iadd to handle the possibility of a carry happening and
affecting the top bits.

However, NIR constant data is always uploaded adjacent to the shader
assembly, in the same buffer. These buffers are required to live in a
4GB region of memory starting at Instruction State Base Address. We
always place the base address at a 4GB address. So the constant data
always lives in a buffer entirely contained within a 4GB region, which
means any offsets from the start of the buffer cannot possibly affect
the high bits.

So instead, we can simply do a 32-bit addition between the low bits of
the base and the offset, then pack that with the unchanged high bits.

On anv, INSTRUCTION_STATE_POOL_MIN_ADDRESS is 8GB, so the high bits are
always 0x2. We don't even need to patch that portion of the address and
can just use an immediate value. We do still need to pack, however.

fossil-db on Icelake indicates the following for affected shaders:

   Instrs: 10830023 -> 10750080 (-0.74%)
   Cycles: 1048521282 -> 1046770379 (-0.17%); split: -0.33%, +0.16%

shader upload time because it's sitting at the end of the shader. This
commit changes ANV to use patch constants to embed the address in the
shader patch the right address in at upload time. This allows us to
avoid having to set up a UBO binding on-the-fly for shader constants.

This commit uses an A64 message but it's quite possible that we could
also use an A32 message and make the dataport do the 64-bit add for us.
However, load_global is what we have right now so it was easier to just
use that.

62bfc700f7 vulkan/util: add struct vk_pipeline_cache_header
Header is defined at vkGetPipelineCacheData spec, in any vulkan
version, and anv, tu and radv were using the same struct, and v3dv was
about to do the same.

Defining the same struct four times seemed odd, so let's define on a
common place.
