Setting variable names currently always uses ralloc, but the new
nir_variable_* helpers will mostly eliminate ralloc/malloc in a later
commit.
This just updates all places that touch nir_variable names to use the new
helpers.

In the C23 standard unreachable() is now a predefined function-like
macro in <stddef.h>

So don't redefine it with the same name, but use the name UNREACHABLE()
to also signify it's a macro.
Using a different name also makes sense because the behavior of the
macro was extending the one of __builtin_unreachable() anyway, and it
also had a different signature, accepting one argument, compared to the
standard unreachable() with no arguments.
This change improves the chances of building mesa with the C23 standard,
which for instance is the default in recent AOSP versions.
All the instances of the macro, including the definition, were updated
with the following command line:

with vtn_bindgen2 we only care about a single function at a time, not a whole
nir_shader, and it would be quite wasteful to serialize all the shader info
every time. add a specialized serialize just for 1 function.

Annotating ssa defs without affecting compilation is impossible with
debug info instructions since referencing a nir_def from the debug info
instr will add uses.
The old approach also stops worrking if passes reorder instructions.
This patch proposes a solution which should not regress performance just
like the old approach. The difference is that this one allocates a bit
more space for debug info instead of adding a new instruction for it.

NIR doesn't really support global instructions such as global val
initilisation. So here we add functionality to glsl_to_nir() to
put these instructions into a temporary function that will be
later inlined into main.
We give the function a name starting with gl_mesa_tmp_ as functions
starting with gl_ are reserved and will not have any clashes with
user functions, we finish the name with the blake3 of the shader
source to avoid conflicts with multiple shaders attached to a single
stage.

The static assert used in encode deref modes used the fact there was
less than 16 modes that we wanted to compress as an opportunity to reuse
MODE_ENC_GENERIC_BIT as it just happened to represent 16. However if we
add more than 16 modes i.e need to compress to 6 bits not 5 bits then
MODE_ENC_GENERIC_BIT becomes 32 and the logic in the assert breaks.
Instead we more precisely make sure MODE_ENC_GENERIC_BIT is large
enough to fit all but the last 4 generic modes and that the last 4 modes
defined in the enum are in fact the 4 generic modes.

With float_controls2, shaders can decide on the behavior of
NaN/Inf/SignedZero preservation by decorating specific instructions, on
top of having a default for the whole program.
Add where to track these to nir_alu_instr and propagate them to new
instructions everywhere that exact is being done already.

We could add a nir_def_num_components() helper but we use
ssa.num_components about 3x as often as nir_dest_num_components() today
so that's a major Coccinelle refactor anyway and this doesn't make it
much worse. Most of this commit was generated byt the following
semantic patch:
