Extra validation is added to ir_validate to make sure this is
always updated to the correct numer of operands, as passes like
lower_instructions modify the instructions directly rather then
generating a new one.
The reduction in time is so small that it is not really
measurable. However callgrind was reporting this function as
being called just under 34 million times while compiling the
Deus Ex shaders (just pre-linking was profiled) with 0.20%
spent in this function.

It doesn't make sense to prefix them with 'image' because
they are called "Memory Qualifiers" and they can be applied
to members of storage buffer blocks.

Having it here rather than in gl_linked_shader allows us to simplify
the code.
Also it is error prone to depend on the gl_linked_shader for programs
in current use because a failed linking attempt will free infomation
about the current program. In i965 we could be trying to recompile
a shader variant but may have lost some required fields.

When a UBO reference has the form block_name.foo where block_name refers
to a block where the first member has a non-zero offset, the base offset
was incorrectly added to the reference.
Fixes an assertion triggered in debug builds by
GL45-CTS.enhanced_layouts.uniform_block_layout_qualifier_conflict. That test
doesn't properly check for correct execution in this case, so I am also
going to send out a piglit test.

There are two distinctly different uses of this struct. The first
is to store GL shader objects. The second is to store information
about a shader stage thats been linked.
The two uses actually share few fields and there is clearly confusion
about their use. For example the linked shaders map one to one with
a program so can simply be destroyed along with the program. However
previously we were calling reference counting on the linked shaders.
We were also creating linked shaders with a name even though it
is always 0 and called the driver version of the _mesa_new_shader()
function unnecessarily for GL shader objects.

Although the glsl_types.h stores this in a bitfield,
we should hide that from everyone else. Hide the cast
in an accessor method and use the enum everywhere.
This makes things a bit nicer in gdb, and improves type
safety.

piglit regressions.

This prevents array overflow when the block is actually an array of UBOs or
SSBOs. On some hardware such as i965, such overflows can cause GPU hangs.

Currently any access params (coherent/volatile/restrict) are being lost
when lowering to the ssbo load/store intrinsics. Keep track of the
variable being used, and bake its access params in as the last arg of
the load/store intrinsics.
If the variable is accessed via an instance block, then 'variable'
points to the instance block variable and not the field inside the
instance block that we are accessing. In order to check access
parameters for the field itself we need to detect this case and keep
track of the corresponding field struct so we can extract the specific
field access information from there instead.
