Extracting from an SGPR could cause a wrong RegClass on
the operand which could later lead to selecting VOPD
instructions which falsely operate on the corresponding
VGPR.

In the C23 standard unreachable() is now a predefined function-like
macro in <stddef.h>

So don't redefine it with the same name, but use the name UNREACHABLE()
to also signify it's a macro.
Using a different name also makes sense because the behavior of the
macro was extending the one of __builtin_unreachable() anyway, and it
also had a different signature, accepting one argument, compared to the
standard unreachable() with no arguments.
This change improves the chances of building mesa with the C23 standard,
which for instance is the default in recent AOSP versions.
All the instances of the macro, including the definition, were updated
with the following command line:

We don't have v_permlane64_b32 yet, but we can still optimize it using
shared vgprs. Using the DPP16 row mask, we can even avoid writing exec.
With v0 input/output and v24/v25 as shared vgprs, this results in:

Sadly amd only made this behavior controlable with global state.
We add a new pseudo opcode for this purpose and change FP16_OVFL
for each instruction. Ideally we would only do it once for clauses
and after ilp scheduling, but this can be improved in the future.

Also ensure that 'needs_scratch_reg' is always true if SCC might be overwritten.
Few changes, because some p_split_vector get SCC as scratch reg assigned,
and thus, can inhibit some postRA optimizations.

Propagated debug info using p_debug_info and Program::debug_info.
Offsets into the shader binary are gathered during assembly.
This will be usefull for mapping back the disassembled shader to
nir, glsl or spirv.

v_swap_b16 is not offically supported as VOP3, so it can't be used with v128-255.
Tests show that VOP3 appears to work correctly, but according to AMD that should
not be relied on.
