We can just place the set structures inside nir_block.
This reduces the number of ralloc calls by 6.7% when compiling Heaven
shaders with radeonsi+ACO using a release build (i.e. not including
nir_validate set allocations, which are also removed).

gc_ctx uses a slab allocator. This reduces GLSL compile times by 1-3%
with the gallium noop driver.
This reduces the number of ralloc_size calls for Heaven shaders by 14.3%.
Note that gc_ctx also uses ralloc_size, so the reduction is a net change.

Store small names in a fixed-sized string in nir_variable.
GLSL IR does the same thing.
When compiling my shader-db with the gallium noop driver, it improves GLSL
compile times by 0.7% (much lower than anticipated).
For Unigine Heaven shaders:
- it eliminates 95.6% ralloc calls for nir_variable names
- the total number of ralloc calls is reduced by 11%
It also adds only 16B to nir_variable, while just the ralloc header
for the name would occupy 40B.

Setting variable names currently always uses ralloc, but the new
nir_variable_* helpers will mostly eliminate ralloc/malloc in a later
commit.
This just updates all places that touch nir_variable names to use the new
helpers.

Most of the time with nir_def_rewrite_uses_after, you want to rewrite after the
replacement. Make that the default thing to be more ergonomic and to drop
parent_instr uses.
We leave nir_def_rewrite_uses_after_instr defined if you really want the old
signature with an arbitrary after point.

So don't redefine it with the same name, but use the name UNREACHABLE()
to also signify it's a macro.
Using a different name also makes sense because the behavior of the
macro was extending the one of __builtin_unreachable() anyway, and it
also had a different signature, accepting one argument, compared to the
standard unreachable() with no arguments.
This change improves the chances of building mesa with the C23 standard,
which for instance is the default in recent AOSP versions.
All the instances of the macro, including the definition, were updated
with the following command line:

When lowering mediump to 16 bits, this will allow drivers to enable
the lowering only for certain pairs of stages, e.g. a driver can lower
mediump for VS->FS, but not GS->FS.
This could also be useful for other things.

This runs validation even if the pass makes no progress. It also requires
all kinds of metadata before the pass to test whether it correctly
preserves or invalidates them.
It's disabled by default because it can be extremely slow.

Annotating ssa defs without affecting compilation is impossible with
debug info instructions since referencing a nir_def from the debug info
instr will add uses.
The old approach also stops worrking if passes reorder instructions.
This patch proposes a solution which should not regress performance just
like the old approach. The difference is that this one allocates a bit
more space for debug info instead of adding a new instruction for it.
