We've regressed the clang-formatting in a few places, since we're not enforcing
formatting in CI yet and I think at one point my editor wasn't quite right.
Reapply so we can get to clang-format-clean.

Minimum/maximum LOD and LOD bias are unsigned and signed fixed point formats
respectively. They are not unsigned integers. Introduce fixed-point types into
our GenXML and use them in the XML, rather than packing in sidebands. This makes
the XML more correct and fixes pretty-printing of texture and sampler
descriptors.

Connor Abbott wrote a nice explanation of how instance divisors work on Mali.
Let's add it to the driver docs instead of letting it languish in a forgotten
header file.
This is mostly pasted from the existing header in tree, with a few local changes
applied.

Nowadays, formats are defined with GenXML, not the old panfrost-job.h, so most
of the format #defines in panfrost-job.h are unused. That said, a few are still
in use as a backdoor for compressed format queries to avoid a GenXML dependency.
That's not great but cleaning that up isn't the subject of this MR.

This switches us over to Mesa's code style [1], normalizing us within the tree.
The results aren't perfect, but they bring us a hell of a lot closer to the rest
of the tree. Panfrost doesn't feel so foreign relative to Mesa with this, which
I think (in retrospect after a bunch of years of being "different") is the right
call.
I skipped PanVK because that's paused right now.

These "quirks" are common for Midgard, yet are only consumed by
pan_lower_framebuffer -- a Midgard-only pass. So the quirks should be removed
and inlined into their users. Thid removes MIDGARD_QUIRKS altogether.

The hardware can handle much larger textures than we allowed. The game
"Cathedral" requires larger textures for some bizarre reason. Raise the
limit so the game runs, syncing MAX_MIP_LEVELS, the comments, and the CAPs.

We only do that when there are more than one damage rectangle and the
number of tiles to reload is significantly lower than the total number
of tiles covered by the damage extent, otherwise the overhead of the
TEM read might defeat the optimization that we might get from using one.

The fixed-function blend logic uses the following equation: A + B x C.
A, B and C are configurable and can be complemented with negation (for
A and B) or inversion (for C) modifiers. Let's rework the blending
code to take that into account.
Note that we need to update the checksum of a few traces because the
equations we use have changed, leading to small deviations on the
final images. Indeed, there are several valid options for a given GL
blend equation, but the operand selection probably has an impact on the
rounding, leading to those mismatch.

There's a lot of code here since the meaning of this field changes
depending on shader state. The good news is that our careful handling
allows preload registers to be decoded now, which pandecode could not
previously do. Likewise, the cmdstream code to emit this is now much
more obvious.
