99f1b7d732 compiler/glsl: explicitly delete copy constructor and copy-assign
Most of our visitors have custom destructors because they wrap C types,
and need to manually call destructors for those types. Because of this,
the implicitly generated copy constructors and copy-assignment operators
are not safe due copying pointers rather than data. Since we don't need
these features just delete them, so any attempt to use them would be a
compilation error. This doesn't fix any existing issue except stopping
coverity from complaining, but it does prevent new issues in the future.

bc0178af57 glsl: don't tree graft globals
As per this optimisations description:

"Takes assignments to variables that are dereferenced only
once and pastes the RHS expression into where the variables
dereferenced."

However the optimisation is run at compile time before multiple
shaders from the same stage could have been pasted together.
So this optimisation can incorrectly assume a global is only
referenced once since it cannot see the other pieces of the
shader stage until link time.

Here we skip the optimisation if the variable is a global. We
could change it to only run at link time however this
optimisation is only run at link time if we are being forced
to use GLSL IR to inline a function that glsl to nir cannot
handle and this will also be removed in a future patchset.

b08aee305e glsl: consistently use ifndef guards over pragma once
Through the glsl headers we had an odd mix of guards be that
"ifndef", "pragma once" neither or both.

Simplify things by using the more common ones (ifndef) and annotating
all the sources, barring the generated builting header -
builtin_int64.h.

The final header - udivmod64.h - is [seemingly] unused and on its way
out (patch purge it is on the mailing list).

fbc1a4b7d2 glsl: Add some comments to methods of ir_variable_refcount_visitor
It was not obvious from the just the .h file what the hash table
contained.  It was also not obvious that get_variable_entry would create
a new entry in the hash table.
