v2: Use 'int main(void)' instead. Suggested by Jason.

f4638cfdad anv/tests: Don't rely on assert or changing NDEBUG in tests
This is the last part of the fix for #2903.

d11e4738a8 anv/allocator: Add a start_offset to anv_state_pool
This allows a pool's allocations to start somewhere other than the base
address. Our first real use of this will be to use a negative offset
for the binding table pool to make it so that the offset is baked into
the pool and the code in anv_batch_chain.c doesn't have to understand
pool offsetting.

70e8064e13 anv: Add an anv_physical_device field to anv_device
Having to always pull the physical device from the instance has been
annoying for almost as long as the driver has existed. It also won't
work in a world where we ever have more than one physical device. This
commit adds a new field called "physical" to anv_device and switches
every location where we use device->instance->physicalDevice to use the
new field instead.

cc972d72c7 anv/tests: Initialize the BO cache and device mutex
We're about to start depending on the BO cache in the state and block
pools so we need them properly initialized for the tests to work.

Some of the tests were actually relying on some of those uninitialized

d3ed72e2c2 anv/allocator: Embed the block_pool in the state_pool

49ecaf88d1 anv/allocator: Drop the block_size field from block_pool
Since the state_stream is now pulling from a state_pool, the only thing
pulling directly off the block pool is the state pool so we can just
move the block_size there. The one exception is when we allocate
binding tables but we can just reference the state pool there as well.

The only functional change here is that we no longer grow the block pool
immediately upon creation so no BO gets allocated until our first state
allocation.
