d31db877e2 util/simple_mtx: fix duplicate definition
botched #ifdef in the cited commit caused simple_mtx_t to be defined
twice in certain cases, which broke the docs build.

cb31b5a958 clc,libcl: Clean up CL includes
This patch does a couple of things to make CL integration with drivers
as seamless as possible:
- We pull in opencl-c.h and opencl-c-base.h to stop relying on system
  headers.
- Parts of libcl.h are moved to new headers that are incomplete CL-safe
  variants of libc headers.
- A couple of util headers are changed to remove now unnecessary
  __OPENCL_VERSION__ guards and make more headers CL safe.
- Drivers now include src/compiler/libcl and use headers like
  macros.h,u_math.h instead of libcl.h.

296eb2918c util: Remove the need of _MTX_INITIALIZER_NP in simple_mtx.h
The usage of _MTX_INITIALIZER_NP is a problem. Sure, the code builds on
Linux and Windows, and it even usually works, but is problematic.
It means that Windows lock debugging tools cannot be used with that code.
So we remove _MTX_INITIALIZER_NP to gain the following benefit:
  Aligning to the C11 standard threads.h definitions.
  Improving portability of the threading code to better support Windows.
This change removes one of the most prolific uses of _MTX_INITIALIZER_NP by
slightly increasing the cost of simple_mtx_lock when targeting platforms that
don't support futex, but it's cost are far less than the cost of syscall,
so the affect are negligible.

The futex code path are not affected. And Windows 8 and upper also
supports the futex codepath, so it's only affect the Windows version < 8, MacOS
or other platforms that doesn't support for futex.

064ec944c1 util: Add extern "C" guard in simple_mtx.h
This is a header that can be accessed both in C/C++,
And when remove the usage of _MTX_INITIALIZER_NP in latter commits, it's need
implement simple_mtx_init in .c file, so for ABI consistence
Add the extern "C" for cpp files

Also add comment for #endif guard for code readability

74665eaf3a util: Detect use-after-destroy in simple_mtx
This makes simple_mtx_destroy set the counter to an invalid canary
value and then makes lock/unlock assert that the value is legal.

That way, calling lock/unlock after destroy will assert fail,
rather than deadlocking or potentially even working.

This has caught real deadlocks in dEQP multithreaded tests (in st/mesa
shader variant zombie list handling), which have since been fixed.

f98a2768ca mesa: Add new fast mtx_t mutex type for basic use cases
While modern pthread mutexes are very fast, they still incur a call to an
external DSO and overhead of the generality and features of pthread mutexes.
Most mutexes in mesa only needs lock/unlock, and the idea here is that we can
inline the atomic operation and make the fast case just two intructions.
Mutexes are subtle and finicky to implement, so we carefully copy the
implementation from Ulrich Dreppers well-written and well-reviewed paper:

We implement "mutex3", which gives us a mutex that has no syscalls on
uncontended lock or unlock.  Further, the uncontended case boils down to a
cmpxchg and an untaken branch and the uncontended unlock is just a locked decr
and an untaken branch.  We use __builtin_expect() to indicate that contention
is unlikely so that gcc will put the contention code out of the main code
flow.

A fast mutex only supports lock/unlock, can't be recursive or used with
condition variables.  We keep the pthread mutex implementation around as
for the few places where we use condition variables or recursive locking.
For platforms or compilers where futex and atomics aren't available,
simple_mtx_t falls back to the pthread mutex.

The pthread mutex lock/unlock overhead shows up on benchmarks for CPU bound
applications.  Most CPU bound cases are helped and some of our internal
bind_buffer_object heavy benchmarks gain up to 10%.
