Without these fixes, H.265 streams using long-term references would
fail to decode correctly as the decoder wouldn't distinguish between
short-term and long-term reference frames.

An H.265 SPS can contain multiple short-term reference picture sets.
Fix the code to properly store and copy all sets instead of just one.

H265 specification defines default scaling lists to use whenever scaling lists
are not specified in neither sps nor pps. Currently drivers ignore this
requirement and set the lists to zero. This commits adds a helper function
vk_video_derive_h265_scaling_list (similar to its h264 counterpart) that
selects either sps or pps lists and falls back to default values if neither
were specified. The default values were taken from ITU-T H265 specification
(revision 8), section 7.4.5.

b-frames can be considered as reference, so the NAL type
should refer to reference type and either RASL or TRAIL
depending on the irap_pic_flag.

driver provided parameters with an encoded bitstream where it has made
changes.
This is the support code to encode the bitstream headers for h265
from parameters (vps/sps/pps).

The H264 spec defines a complicated way of layering PPS scaling lists
on top of SPS scaling lists. The details can be found in 7.4.2.1
(seq_scaling_matrix_present_flag semantics) and 7.4.2.2
(pic_scaling_matrix_present_flag semantics).
Both ANV and RADV need to derive the final scaling lists sent to HW
using this logic in order to handle H264 scaling lists correctly.

The video session and video session parameters objects can have a common
base class the drivers can inherit from if needed.
This creates code to parse the h264/h265 parameter sets into common
structs.
v2: add h265 VPS, add a macro for FIND/ADD generations, changes the API
to make generation easier.
