While analyzing cache loading performance, hashing the pipeline layout was
surprisingly consuming around 4% of time, sometimes close to the cost of
hashing shader modules.
Turns out we were hashing the pipeline layout on every pipeline creation.
Considering that pipeline layouts are usually deduplicated by the
application, this was amplifying the hashing cost by a big margin.
With Graphics Pipeline Library, we do need to rebuild the pipeline layout
by combining those from each library, but we can memoize the hash of the
descriptor set layout. The cost of re-hashing hashes is negligible since
each descriptor set layout can amount to 1–2KB in size.

It's unnecessary and already checked elsewhere like in
vkCmdBindDescriptorSets(). This improves performance of vkoverhead
test #76 (descriptor_1image) by +18%. It's the same performance as
PRO on my Threadripper 1950X now. This should also slightly improve
texel and buffer descriptors.

Separately track descriptor set layouts for pools with
FREE_DESCRIPTOR_SET disabled, instead of reusing
`radv_descriptor_pool::entries`. This saves 8 bytes for each entry and
also avoids some indirection.

Previously this check was skipped for pools with
VK_DESCRIPTOR_POOL_CREATE_FREE_DESCRIPTOR_SET_BIT unset, but after
96a240e1 we need to check this otherwise we risk overflowing
radv_descriptor_pool::entries into the host memory base
This fixes a crash to desktop when launching Dota 2, which overallocates
descriptor sets and expects an error to allocate another descriptor pool

The spec states that descriptor set layouts can be destroyed almost
at any time:
"VkDescriptorSetLayout objects may be accessed by commands that
operate on descriptor sets allocated using that layout, and those
descriptor sets must not be updated with vkUpdateDescriptorSets
after the descriptor set layout has been destroyed. Otherwise,
descriptor set layouts can be destroyed any time they are not in
use by an API command."

Uplay needs this to avoid a crash because it does an use-after-free
of a descriptor set layout. This was initially introduced by Bas to
workaround a similar issue with Baldur's Gate 3, it seems needed again.
