In the C23 standard unreachable() is now a predefined function-like
macro in <stddef.h>

And this causes build errors when building for C23:

So don't redefine it with the same name, but use the name UNREACHABLE()
to also signify it's a macro.
Using a different name also makes sense because the behavior of the
macro was extending the one of __builtin_unreachable() anyway, and it
also had a different signature, accepting one argument, compared to the
standard unreachable() with no arguments.
This change improves the chances of building mesa with the C23 standard,
which for instance is the default in recent AOSP versions.
All the instances of the macro, including the definition, were updated
with the following command line:

This commit moves the calculation of tri_bary out of
brw_nir_rt_load_mem_hit_from_addr(), and only do the calculation on
demand, since unorm_float_convert can be expensive. We do this for both
Xe1/2 and Xe3+ for consistency.

While the documentation says to use NUM_SIMD_LANES_PER_DSS for the stack
address calculation, what the HW actually uses is
NUM_SYNC_STACKID_PER_DSS. The former may vary depending on the platform,
while the latter is fixed to 2048 for all current platforms.

Instead, we replace every use of it with nir_def. Most of this commit
was generated by sed:
sed -i -e 's/dest.ssa/def/g' src/**/*.h src/**/*.c src/**/*.cpp
A few manual fixups were required in lima and the nir_legacy code.

Initially I tried to store ray query state in the RT scratch space but
got the offset wrong. In the end putting this in the scratch surface
makes more sense, especially for non RT stages.

We need the traversal stack to saved/restored along with mem hits.
Total spill/fill is 256bytes.
We can potentially optimize this but we have to be very careful about
what state the query is in.
