Since 624e799cc3 ("nir: Drop nir_ssa_def::name and nir_register::name"), SSA
defs don't have names, making the name argument unused. Drop it from the
signature and fix the call sites. This was done with the help of the following
Coccinelle semantic patch:

The shaders affected here are ones that were previously affected when
i2b was unconditionally lowered in opt_algebraic. There are a few places
where some transformations happen in a different order, so some
algebraic patterns are missed.

If both types are the same or both are integer types with the same bit
size, no actual conversion happens and nir_type_conversion_op() will
return nir_op_mov. In this case, there's no point in emitting the move
and we can just return src instead.

Many users of nir_vec() do so by nir_channel()-ing a new ssa defs as movs
from other vectors to put the new vector together, which then just have to
get copy-propagated into the ALU srcs and DCEed away the temporary movs.
If they instead take nir_ssa_scalar, we can avoid that extra work.

This saves a bunch of generated code to pack up the extra NULLs to get to
4 args, and saves executing the conditions in nir_build_alu() to then skip
those NULLs.
Saves another 27kb on disk.

This function is big and I don't think it will won't get meaningfully
constant-propagated during inlining without LTO. Move it to a .c file so
we just have one copy, saving 2.8MB from libnir.a on an amd64 release
build.
