This will be important for residency in a future change, but also
is necessary for synchronize() to work correctly for TBOs.

There's already a single command queue for the screen, meaning that
all commands are being serialized implicitly into that queue. There's
no need to have separate fences for parallel contexts when those
fences would all share the same underlying timeline.
This adds an explicit lock to expand the scope of the implicit screen
command queue ordering to include fence signals.
Each context still gets its own submit sequence, which is used for 1
purpose right now: A uniqueness check in the state manager to see
if states are coming from separate command lists, to apply promotion
and decay logic.

This is a bit fragile. The algorithm is essentially:
- Let the driver track state for non-dual-bound resources.
- For resources that are dual-bound as SSBO/image and a second
bind point, assume they're being used as UAVs.
- When a MemoryBarrier is issued, dirty all destination bind points
so they re-assert their state, and if the destination is not UAV,
temporarily suppress UAV state re-assertion.

I'd assumed that since insert didn't take a deleter, it was
find-or-insert, not insert-or-replace. This caused a bo reference
leak if the same bo was used more than once in a batch.

We've been inconsistent between IID_PPV_ARGS,
__uuidof(var), and __uuidof(type). Since Linux doesn't
support the latter of these, they need to be changed.
While we're at it, switch all __uuidof to the more terse
IIV_PPV_ARGS option.

MSVC has an extension for getting IIDs (GUIDs) from types. Other
compilers can support this extension when targeting Windows, but
don't support it when targeting Linux. Instead, winadapter.h
defines __uuidof(var) to uuidof<decltype(var)>. Then dxguids.h
provides inline specialized definitions for the known D3D types.

This driver will allow running OpenGL and OpenCL on top of Gallium
for any hardware supporting Microsoft's Direct3D 12 on Windows 10.
This is the combination of a lot of commits from our development branch,
containing code from several authors.
