a5f9e59ce3 anv: Use vma_heap for descriptor pool host allocation
Pre-patch, anv_descriptor_pool used a free list for host allocations
that never merged adjacent free blocks. If the pool only allocated
fixed-sized blocks, then this would not be a problem. But the pool
allocations are variable-sized, and this caused over half of the pool's
memory to be consumed by unusable free blocks in some workloads, causing
unnecessary memory footprint.

Replacing the free list with util_vma_heap, which does merge adjacent
free blocks, fixes the memory explosion in the target workload.

Disdavantges of util_vma_heap compared to the free list:
  - The heap calls malloc() when a new hole is created.
  - The heap calls free() when a hole disappears or is merged with an
    adjacent hole.
  - The Vulkan spec expects descriptor set creation/destruction to be
thread-local lockless in the common case. For workloads that
create/destroy with high frequency, malloc/free may cause overhead.
    Profiling is needed.

Tested with a ChromeOS internal TensorFlow benchmark, provided by
package 'tensorflow', running with its OpenCL backend on clvk.

  cmdline: benchmark_model --graph=mn2.tflite --use_gpu=true --min_secs=60
  gpu: adl
memory footprint from start of benchmark:
    before: init=132.691MB max=227.684MB
    after: init=134.988MB max=134.988MB

4ceaed7839 anv: split internal surface states from descriptors
On Intel HW we use the same mechanism for internal operations surfaces
as well as application surfaces (VkDescriptor).

This change splits the surface pool in 2, one part dedicated to
internal allocations, the other to application VkDescriptors.

To do so, the STATE_BASE_ADDRESS::SurfaceStateBaseAddress points to a
4Gb area, with the following layout :
   - 1Gb of binding table pool
   - 2Gb of internal surface states
   - 1Gb of bindless surface states

That way any entry from the binding table can refer to both internal &
bindless surface states but none of the driver allocations interfere
with the allocation of the application.

Based off a change from Sviatoslav Peleshko.

b49b18f0b7 anv: reduce BT emissions & surface state writes with push descriptors
Zink on Anv running Gfxbench gl_driver2 is significantly slower than
Iris.

The reason is simple, whereas Iris implements uniform updates using
push constants and only has to emit 3DSTATE_CONSTANT_* packets, Zink
uses push descriptors with a uniform buffer, which on our
implementation use both push constants & binding tables.

Anv ends up doing the following for each uniform update :
   - allocate 2 surface states :
      - one for the uniform buffer as the offset specify by zink
      - one for the descriptor set buffer
   - pack the 2 RENDER_SURFACE_STATE
   - re-emit binding tables
   - re-emit push constants

Of all of those operations, only the last one ends up being useful in
this benchmark because all the uniforms have been promoted to push
constants.

This change defers the 3 first operations at draw time and executes
them only if the pipeline needs them.

87f59b18cf anv: don't lazy allocate surface states in descriptor sets
In 4001d9ce1a we started lazily allocating surface states in the
descriptor sets rather than upfront in the descriptor pool. This was
to workaround vkd3d-proton allocating more than we could handle at the
HW level.

The issue introduced in that change is that we didn't protect the
descriptor pool free list as well as the anv_state_stream which are
now potentially used from different threads through the descriptor set
write functions.

This reverts the lazy allocation part of that change. Host only
descriptor sets changes remain.

This change introduces the anv_descriptor_size_for_mutable_type and
anv_descriptor_data_for_mutable_type helpers to compute the size and
data flags respectively for mutable descriptor types.

In order to make handling these types easier we now store a precomputed
descriptor stride for all types and use in the in appropriate places.

We also need to adjust the compiler to take into account this descriptor
stride. To that extent, we now pack the stride into the upper 16 bits
alongside the index and the dynamic offset index and use it later to
compute the correct offset.

or even to a raw (SSBO) read.

The hardware has always had basically full support for typed writes all
the way back to Ivy Bridge but typed reads have been harder to come by.
Starting with Skylake, we finally have enough that we at least have a
format of the right bit size but not necessarily the right format so we
can use a typed read but may still have to do an int->unorm or similar
cast in the shader.

Previously, in ANV, we treated lowered images as the default and write-
only as a special case that we can optimize. This flips everything
around and treats the cases where we don't need to do any lowering as
the default "vanilla" case and treats the lowered case as special.
Importantly, this means that read-write access to surfaces where the
native format handles typed writes now use the same surface state as
write-only access and the only thing that uses the lowered surface state
is access read-write access with a format that doesn't support typed
reads. This has the added benefit that now, if someone does a read
without specifying a format, we can default to the vanilla surface and
it will work as long as it's a format that supports typed reads.

aa94220d7d anv: Stop printing descriptor pool allocation failures
The VK_ERROR_FRAGMENTED_POOL and VK_ERROR_OUT_OF_POOL_MEMORY errors are
not as exceptional cases as most. These are expected to be hit by
applications in the normal course of doing their thing. Probably best
not to spam stderr and the debug logs with them.
