mesa: Remove prototypes for nonexistent functions

_mesa_UseShaderProgramEXT, _mesa_ActiveProgramEXT, and
_mesa_CreateShaderProgramEXT were all removed when support for
GL_EXT_separate_shader_objects was removed.
