mesa/extensions: Prefix global struct and extension type

Rename the following types and variables:
* struct extension -> struct mesa_extension,
  like the mesa_format type.
* extension_table -> _mesa_extension_table,
  like the _mesa_extension_override_{enables,disables} structs.
